Who Can Qualify
Who Can Qualify
Individuals who recieve Medicaid benefits AND:
- Are 21 years or older
- Meet nursing facility level of care
- At minimum require limited assistance with one or more daily living activities
- Are able to direct their care independently through a responsible representative
- Are in a nursing facility and is able to be discharged if community-based services are available
- Are likely to require nursing facility admission within the next 120 days
- Have a primary caregiver who has a disability or who is at least 70 years old
